	Based on Article. 6 and 9 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC, I declare that I am expressing consent to the processing of my personal data by Morska Agencja Gdynia Sp. z o.o. with headquarters in w Gdynia, ul. Tadeusza Wendy 15, registered under KRS number 35292 with the NIP number 586-015-78-96 and REGON 190549721, which will be their administrator, for current and future activities to mediate in the recruitment and employment process for foreign shipowners, including for the purposes necessary to implement the seafarer employment contract and its financial security, in accordance with the requirements of the MLC 2006 Convention. I declare that I consent to the transfer of personal data to third countries, for which the European Commission has not issued a decision stating an appropriate level of protection pursuant to GDPR art. 45, 3. I declare that I am expressing consent to the processing of my personal data for these purposes voluntarily and I have been informed about the possibility of withdrawing consent at any time.
	Na podstawie art. 6 i 9 Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE oświadczam, że wyrażam zgodę na przetwarzanie moich danych osobowych przez Morska Agencja Gdynia Sp. z o.o.. z siedzibą w Gdyni, ul. Tadeusza Wendy 15, zarejestrowaną pod numerem KRS 35292 posiadającą numer NIP 586-015-78-96 oraz REGON 190549721, która będzie ich administratorem, dla celów obecnych i przyszłych działań w celu pośrednictwa w procesie rekrutacji i zatrudnienia u armatora zagranicznego, w tym do celów niezbędnych do realizacji marynarskiej umowy o pracę i jej finansowego zabezpieczenia, zgodnie z wymaganiami Konwencji MLC 2006. Oświadczam, że wyrażam zgode na przesyłanie danych osobowych do państw trzecich , co do których Komisja Europejska nie wydała decyzji stwierdzającej odpowiedni stopień ochrony na mocy na mocy GDPR art.45, 3. Oświadczam, że wrażam zgodę na przetwarzanie moich danych osobowych w powyższych celach dobrowolnie i zostałem poinformowany o możliwości wycofania zgody w każdym czasie.
